Application Process

Thank you for your interest in the 2008 Youth Exchange. Please note that the selection committee is presently selecting the group volunteers to accompany the selected youth participants.

The following forms are for selected participants only.

YOUTH

3 Forms for General Youth Exchange:

  1. Medical Form
  2. Youth Exchange Consent Form - Youth Participant
  3. Photo Release Form

3 Forms for Rock Climbing:

  1. MultiTrek Rock Climbing Registration Form
  2. MultiTrek Rock Climbing Consent Form - Youth Participant
  3. MultiTrek Rock Climbing Consent Form - Parent/Guardian

GROUP VOLUNTEER

  1. Youth Exchange Leader Consent Form: all group volunteers
  2. Personal release: all group volunteers
  3. Group Volunteer Information: all group volunteers
  4. Group Volunteer Agreement: chaperone = traveling with the youth
  5. Group Volunteer Agreement: leader = not traveling with youth
